Who We Are: The Community Health Center of Burlington is a Federally Qualified Health Center Chittenden County.  Founded in 1971, the Health Center is dedicated to provide a respectful health care home for all residents, regardless of their circumstances.  Our services include the following:

Primary Care : As a family practice, our physicians, physician assistants, and nurse practitioners provide primary and preventive care to all ages. Services include:
Adult Health care
Prenatal care and deliveries
Well Child Care
Acute Injury and Illness Treatment
Convenient Laboratory Services on Site
Anonymous HIV Testing and Counseling (Wednesdays, 3-6:00pm)
Interpreters Available at All Visits
Connection to Free or Low Cost Prescription Assistance Programs

Dental Care   We offer a full range of preventive and restorative dental services to people who live or work in Chittenden County.

Emergency patients are accepted from Chittenden, Franklin, Addison,
Lamoille and Grand Isle counties

Mother and Child Prenatal Program  Moms need care and support to have a healthy baby. Our prenatal program provides special services such as case management from a specially trained nurse and one-on-one social work services that can include tobacco cessation, stress reduction, mental health counseling and help finding other community resources.  After your baby is born, a certified lactation consultant and our family practice physicians will help your new family thrive.
  Mental Health and Substance Abuse Services are offered on site by LICSW social workers with convenient hours in the comfort and anonymity of your family doctor’s office.
  Free Specialty Clinics  Volunteer providers staff free, monthly specialty clinics in neurology and surgery.  Health Center patients can access this specialty care in the welcoming environment of their family doctor.  These volunteers provide care at the Health Center as well as answer patients’ questions should they need to be referred for follow-up care.

Homeless Healthcare Program Since 1988, CHCB has been the sole Vermont grantee for federal Health Care for the Homeless funding. Our special services and medical clinics for men, women, children and teens experiencing homelessness offer the comprehensive care necessary to heal and move on to lives of safety, security and good health.
 

The Safe Harbor Clinic is located at 184 South Winooski Avenue in Burlington. Safe Harbor is open daily for no cost walk-in and appointments visits for men, women and children experiencing homelessness. Services include:
Primary & preventive health care
Preventive & restorative dental care
Case management & mental health counseling with CHCB nurses & licensed clinical social workers as appropriate
Mental health & substance abuse counseling with program partner, Howard Center for Human Services
Outreach to bring people into care
Housing First supported living housing program

The Pearl Street Clinic located at 179 Pearl Street in Burlington is open to homeless, runaway and at-risk youth ages 14-22. No cost, walk-in services include:

  • Primary and preventive health care, including physicals
  • Social worker on site 40 hours a week for case management, connection to benefits and supportive counseling.

 

Want to be a patient? Just call us! We accept most insurances and welcome Medicaid and VHAP, too. If you think you are eligible for our sliding fee scale program, bring in two forms of proofs of income, such as your tax return, last two pay/unemployment stubs or your unemployment benefit page. We welcome people without health insurance As a Federally Qualified Health Center all services are available on a sliding fee scale for low income, uninsured community residents. If your income is too high to be eligible for our sliding fee scale programs, we offer a 25% time of service discount to help make the burden a little easier.
